The Ohio Department of Education has provided Diagnostic Assessment materials to the districts.

Beginning in 2003, sets of these materials were shipped to school districts, one for each classroom in each building. These materials include kindergarten, grade 1 and grade 2 teacher notebooks that contain all screening measures. There are also kindergarten and grade 1 teacher notebooks that contains the diagnostic measures appropriate for that grade. Grades 2 and 3 materials are packages of student test booklets and administrative manuals.

Do you need to get more materials?

The Ohio Department of Education has committed to provide complete sets of these materials for new classrooms in districts for the next several years, and replenish certain items annually. For specifics on acquiring diagnostic materials, please contact the Office of Assessment at 614-466-0223.  The complete set of diagnostic assessment materials are also available online at the ODE website as adobe acrobat files.
